Buffy the Vampire Slayer, "The Body."
I don't think I have ever cried so much for a show/movie/book. That includes "Doomsday," (yes, I cried) "The Wrath of Khan," (yup. cried.) the death of Sirius in book five (cried. Alot.), and what happened with Snape in book seven (I bawled like a baby.)
I have never seen the subject of grief dealt with this way in a show or movie. The episode was beautifully done, and rang absolutely true. The scene where Willow kept freaking out about what to wear, and how Anya broke down because she just couldn't understand it... I just have no words. This goes beyond "heartwrenching."

I think one of the things I found most effective was the complete lack of any sort of music/soundtrack.
(P.S. -I cried at the Wrath of Khan, too.)

Joss Weadon is so brilliant that I would agree to see anything he wrote/directed without asking a single question beyond: "when?"
I, too, cried like a hungry, angry baby at that episode. And Sirius in book 5 and Snape in like... everything. Including most of the fanfics I read, to this day. ><
